“It always seems too early until it’s too late”
Join knowledgeable professionals for National Healthcare Decisions Day Monday, April 16 hosted by Henry Community Health’s Care Coordination team and HCH Hospice.
In an open workshop setting from 9 am to 4 pm in the Education Center at Northfield Park Primary Care Campus in front of the YMCA participants will be informed of critical healthcare decisions they need to make involving such items as advanced directives, living wills, healthcare representatives and more.
“Advanced care planning is the best gift you can give to your family,” said Care Coordination Director Shelley Wilson, RN and organizer of the event. “We want to make this event a hands-on workshop for people of all ages by providing you with tools as well as information.”
Various forms for healthcare decisions will be available along with a notary who will be on site for the convenience of participants.
A coffee bar supplied by Henry Community Health Hospice will be available throughout the event while participants check out:
- Do Not Resuscitate Declaration and Order
- Healthcare representative Appointment
- Life Prolonging Procedures Declaration
- Living Will Declaration
- Physician Orders for Scope of Treatment
