Living Better With Diabetes
Let us help you learn to care for your diabetes whether you are newly diagnosed or have had diabetes for many years. Diabetes education makes you more aware of diabetes, what it takes to treat it, and gives you the power to control it.
Your provider may refer you to the class or you may sign up yourself by calling 521.1544.
Our goal is to teach the skills needed to attain independence in Diabetes Management and activities of daily living.
Sessions focus on diet, medication, psychological aspects, exercise and complications. Instruction and support is provided by a multi-disciplinary team of professionals including a Diabetes Nurse Educator and a Registered Dietitian.
Group classes are offered monthly once a week for four weeks at Henry Community Health’s Primary Care Campus at Northfield Park in the Education Center.
4 Consecutive Mondays
January 10 am-noon
February 1-3 pm
March 6-8 pm
Registration is Required Prior to Attending
Make-up sessions are available. Patients are encouraged to bring a family member or significant other with them to class.

- General knowledge of diabetes
- Meal planning
- Self monitoring of blood sugar
- Stress management
- Prevention and treatment of acute and chronic complications
- Community resources
- Role of education
- Individual Counseling
