Live Better With Diabetes
Let us help you learn to care for your diabetes whether you are newly diagnosed or have had diabetes for many years. Your provider may refer you to any of these programs or you can refer yourself.
Diabetes education makes you more aware of diabetes, what it takes to treat it, and gives you the power to control it. Henry Community Health offers many opportunities for diabetic education.
We have a variety of education programs available to help you learn skills to manage your diabetes. To learn more about our current programs contact Steve Hamilton, Diabetes Educator, at 765.521.1544.
Our free classes offer the expertise of our diabetes educator, a dietitian, pharmacist, and a nurse specializing in wound care from the Advanced Wound Center. The classes meet weekly for one month and teach participants about diabetes self-management. For more information call 521.1544.
Diabetes Support Group
Our Diabetes Support Group meets at 1:30 pm on the second Thursday of the month in the Education Center at the HCH Primary Care Campus at Northfield Park.
Counseling sessions are offered at no charge by our Diabetes Educator to help you understand, control and live well with diabetes. There is a fee for diet instruction with a Registered Dietitian.
Women with gestational diabetes will be scheduled to meet with our Diabetes Educator by Antolin & Benninger Obstetrics/Gynecology.
