Accessing your imaging results
Imaging records are now available through Clearpath! Instead of picking up a CD, you can access your records from the comfort of your home and instantly share with any physician or family member.

How to get Clearpath
If you have had imaging performed at Henry Community Health, you may request your imaging records online through the Clearpath portal.
To request records, please follow the steps below.
- Create an Account at https://patient.myclearpath.com/henry
- You will be asked to enter your first and last name, date of birth, e-mail address, and time zone.
- Once you create an account, a temporary password will be sent to your e-mail.
- Use the temporary password and your e-mail address to log in
- Request your records
How to request records
- Select the Request Record Tab
- To request a record, select the Request Record button on the bottom right corner
- Select Record Custodian
- If you do not see Henry Community Health’s name listed, please reach out to help@myclearpath.com.
- Select records to request, body part, and date range
- Agree to the terms and conditions and submit
Prior to requesting your records, please ensure your Photo ID and signature are attached to your profile under the My Identity Tab
Once submitted, Henry Community Health will review and process. Once request is processed, if approved you will see your images under the My Records Tab
