Reschedule or cancel
Change of plans? Please give us 24 hours’ advance notice. Call your provider’s office directly or reply to your confirmation text to update your scheduled visit.

Prepare for your visit
- Arrive 15 minutes before your appointment time to fill out paperwork and complete any check-in procedures.
- To reschedule, please give us 24 hours’ notice.
- In the event of an emergency or last-minute need, call the office as soon as possible.
Note: For the safety of our young patients, all minors need to be accompanied by an adult.
What to bring
- Photo ID
- Insurance card
- List of current medications
- List of your other providers
- Imaging or test results if you have them
- Medical records if you have them
Appointment check-in
- You’ll start each visit at patient registration, where one of our representatives will guide you through the check-in process.
- If visiting the hospital, we’ll provide you with a wristband that helps our team match you with the right care—from medications to lab work.
FAQs about appointments
If you or your practice would like to refer a patient to Henry Community Health for evaluation or treatment, we would be happy to coordinate care. Visit our referrals page for future details.
If you’re a patient who needs specialized care, you may need your primary care physician to make a referral. Contact your insurance provider to find out whether a physician referral is required.
Your time matters to us, and so does the time of other patients.
As soon as you know you can’t make your scheduled visit, please call your provider’s office. This early notice allows us to offer your appointment time to another patient who needs care. Any appointments missed, canceled, or rescheduled with less than 24 hours’ notice will be considered “no show”.
