Our regular business office hours are Monday through Friday 8 am-5 pm.

Please note that medication refills and appointment changes are only handled during regular business hours.

If it is a medical emergency and you need to speak to a Provider on call, please call 599-3400.
Patient needing care that is 18 years and older, Press 1
Patient needing care that is 17 years and younger, Press 2.
Our answering service will relay your information to the provider on call who will contact you.

If it is not an emergency, please call during our regular business hours.

Insurance & Payment

Our practice accepts most insurance, Medicaid and Medicare. Patients with co-pays are expected to pay their co-payment at the time of service. Patients without insurance or who need assistance should contact Henry Community Health Business office at 765.521.1516. Payment may be made by check, cash or credit/debit card. We accept Visa, MasterCard and Discover.

Personal Information

Please let staff know if there has been any change in your personal information such as name, address, or phone number. You also need to let us know if information about your health care needs can be shared with a family member(s) and if we can leave information on an answering machine. During your initial visit you will be asked to fill out this information. However, please let us know if your wishes change at any time regarding these personal privacy issues.

Hospitalization

We use the Adult and Pediatric Hospitalist programs if you need to be hospitalized at Henry Community Health. The Hospitalists are members of Henry Community Health. They practice full-time in the Hospital which allows them to spend more time on your care while you are a patient. Please be assured they have our full confidence in providing your hospital care, communicating with you and your family and keeping us informed about your progress. We will resume your care after your hospital discharge.

Referrals

If our provider determines you need to be seen by a specialist, we will make the necessary referral and appointment.  Prior authorizations may take up to 48 to 72 hours to process. We will make every attempt to ensure that your referral is to a provider or healthcare facility that is considered in network by your health insurance plan. However, because these networks are constantly changing, it is extremely important for you to contact your insurance company to verify that the provider and/or facilities are still considered in network so you will not pay a penalty for seeing an out of network provider.

Reach Out and Read

Growing up healthy means growing up with books. Our practice participates in Reach Out and Read® which is a national non-profit organization that encourages reading by giving new books to children and advice to parents about the importance of reading aloud to their children. Access and exposure to books beginning in infancy is critical for a child’s development as well as their success in school and in life. In our program, your child will receive new, age appropriate books during your child’s regular well child check from six months through five years of age.
