As a patient of Henry Community Health (including the hospital, physician offices and outpatient services) you can access your health information at any time through the patient portal MyHealtheLife.

If the information you need is not available on MyHealtheLife, you may request a copy of your medical records from our Medical Records Department. To request your medical records from this department, please download and complete the Authorization To Disclose Protected Health Information Form.

There may be a fee for copies of your records obtained through our Medical Records Department.  Fax or mail the completed form along with a copy of your photo ID to Medical Records at:
