The Henry Community Health Medical Records Department is here to support you by keeping your health information organized, secure, and easy to access. It is our job to ensure that your personal information is kept private and safe, so you can feel confident sharing your health details with us. Our goal is to make the healthcare experience smoother and provide patients with easy access to their medical records.

 

 

Imaging results now accessible online

Imaging records are now available through Clearpath! Instead of picking up a CD, you can access your records from the comfort of your home.

Viewing medical records with MyHealtheLife

MyHealtheLife allows you to view your medical records with these quick steps:

  • Go to the patient portal using your device of choice
  • Login with your secure credentials
  • Click on the Health Record tab
  • Choose a document to view

From here, you can view information regarding recent lab results, immunization records and more.

Obtaining your medical records

Your medical information is secured and protected by HCH. To obtain a printed copy of your medical records, please download and complete the HIPAA-compliant Release of Information Authorization Form. Submit the completed form, along with a copy of your valid photo ID, via one of these methods. Our medical records department is here to help get you the information that you are looking for.
