Enrolling in MyHealtheLife

At your next appointment or hospital visit, staff will help you enroll in MyHealtheLife. If you prefer not to wait until then to enroll you may enroll at your physician’s office or at Patient Registration at the hospital. You may contact Joy at 765.599.3576 if you have questions.

Enrollment Process

At check-in, a Care Team member will ask if you would like to provide your email address so you can receive an invitation to enroll in MyHealtheLife. Be ready to:

  • Show your driver’s license or photo ID as part of our patient privacy verification process.
  • Give your email address.
  • Answer security questions as part of the enrollment process for your security protection.

If you want to enroll before you have an appointment or hospital visit:

  • You may enroll at your physician’s office or at Patient Registration at the Hospital.
  • You will be asked to show your driver’s license or other photo ID as part of our patient privacy verification process.
  • A Care Team member will provide you with information on how to self-enroll.

Register Through Your Email Invitation

  • Click the link in your email from Henry Community Health inviting you to join MyHealtheLife.
  • Complete the registration process and set your password.
  • Once registration is complete, information from Henry Community Health will automatically be sent to your portal.

Your invitation will expire after 90 days so please register quickly after you receive the email invitation. You may want to check your spam in case your invitation is not recognized by your software. You will receive a reminder if you haven’t activated the code after 7 days, and then again after 30 days from the time your invitation was sent. These are the only reminders you will receive during the 90-day period.

Using Your Patient Portal

You will receive an email notifying you when new information is available on your portal. This includes information about any visits, lab work etc. and answers to any questions you have sent to your physician or other provider.

Family Members On One Portal

If you want your portal registration to include other family members you will need to use the same email address, username and password for the entire family.

Reminders
