A Wellness benefit for employees covered under the HCH insurance plan!

As part of our health benefits package for employees covered under the HCH insurance plan, we offer a Wellness @ Work program that includes a Biometric Screening (blood pressure, height, weight, cholesterol and glucose), a Health Risk Assessment and Health Coaching.

For your participation and completion of just these three items – screening, assessment and attendance at just one coaching session – you will be awarded four PDO hours. All 3 steps must be met prior to receiving the incentive.

  1. Biometric Screenings: While a screening does not diagnose medical problems, if you have results outside of normal ranges, it is important for you to follow up and make an appointment with your primary care provider to determine what future treatment you might need.
  2. Health Risk Assessment: You will fill out a brief questionnaire regarding your health.
  3. Health Coaching Session: Sessions with a health coach can put you on track to help you lose weight, better manage a chronic disease, stop smoking, lower your cholesterol or blood pressure, reduce stress, eat better and even exercise regularly. Information shared at these sessions is meant to guide you into making healthier choices and does not replace your physician’s advice and/or treatment.

Your Privacy Ensured: Your individual screening results will only be shared with your healthcare provider and the Wellness @ Work team. Hospital Administration will only receive aggregated data reports regarding the overall health of our employee population and no individual names or results.
